When you arrive for your first visit, please be prepared to complete all necessary insurance and health information forms, which will enable us to begin your dental treatment. We will ask you to fill out forms that will help us get acquainted with you and your dental needs. Please be sure to bring a list of your current medications, as well as the phone number of your family doctor and the pharmacy you prefer. If you have an extensive medical history, Dr. Borton may require a medical clearance from your physician. Please notify us when scheduling if you have a history of hip or knee replacement, cardiac stents, heart murmurs, or heart anomalies. You may need to take an antibiotic before dental treatment or discontinue blood thinners.
